What is an AI receptionist? A guide for clinics in India
What is an AI receptionist?
An AI receptionist is a computer system that answers your business phone and talks to callers the way a person at the front desk would. It listens to what the caller says, works out what they need, answers from information you give it, and finishes the task, most often booking, changing or cancelling an appointment. It works at any hour and can be set up to answer in several languages.
It is different from the old phone menus that say "press 1 for appointments". The caller simply talks, and the AI replies in full sentences. Modern systems combine three parts: speech recognition that turns the caller's voice into text, a language model that decides what to say and do, and a voice that speaks the reply.
For a clinic, the practical meaning is simple: calls that would have rung out now end in a booking or a message for the doctor.
How does an AI receptionist handle a call?
A typical call to a clinic's AI receptionist goes through five steps. Each takes a second or two, so the conversation feels close to normal speech.
| Step | What happens | What can go wrong |
|---|---|---|
| 1. Answer | The call is forwarded from the clinic number and answered on the first ring with a greeting. | Forwarding not set up for busy or after-hours calls |
| 2. Listen | Speech recognition turns the caller's words into text, in their language. | Names, numbers and dates misheard, especially with background noise |
| 3. Decide | The AI works out the request: book, reschedule, cancel, a question, or a message. | Answering a question it should pass to the doctor |
| 4. Act | It checks free slots and books, or looks up the answer in the clinic's approved facts. | Double booking if slots are not kept up to date |
| 5. Record | The booking or message is saved, for example to a Google Sheet or clinic software. | Details saved wrongly if a misheard name is not confirmed |
The "what can go wrong" column is where products differ most. Good systems read back names, dates and times to confirm them, and the team behind them checks recordings to fix mistakes. At GrowthBara, every call Priya takes is transcribed again from the recording and compared with what she heard live, so misheard words are caught and fixed.
What can an AI receptionist do for a clinic?
Most of a clinic's phone calls are routine. These are the jobs an AI receptionist handles well:
- Answer after-hours calls. Patients call at night, early morning and on Sundays. The AI answers and books the next free slot.
- Book, reschedule and cancel. It checks the doctor's free slots and makes the change while the caller is on the line.
- Answer common questions. Timings, address, landmark, parking, which doctor is in on which day, and prices you choose to share.
- Take messages. When a caller needs a person, it records their name, number and reason for the doctor or staff.
- Make outgoing calls. Some systems, Priya included, call patients for their next package session or a recall visit.
- Keep a record. Every call ends as a row the clinic can see, so no request is lost in someone's memory.
For examples by specialty, see how this works for dental clinics and for skin and hair clinics.
What can it not do?
Being honest about limits is the best way to decide if one suits you. An AI receptionist:
- Should not give medical advice. It must not diagnose, suggest medicines or say whether a treatment is needed. A well-built one hands these questions to the doctor and tells callers with warning signs to seek emergency care.
- Only knows what you tell it. If your timings change and nobody updates it, it will give old timings.
- Can mishear. Unusual names, poor phone lines and loud waiting rooms cause errors. Confirmation read-backs reduce this but do not remove it.
- Is not right for every call. An upset patient, a complaint or a complicated billing question is better handled by your team. The AI should take a message and make sure someone calls back.
- Depends on the phone network. If the line or the service goes down, calls are missed, just as they are today.
How much does an AI receptionist cost in India?
Prices in India usually follow one of three models: a fixed monthly plan with a set number of included minutes, pure pay-per-minute, or a per-call charge. Some vendors add a one-time setup fee. When comparing, convert every quote to a monthly figure at your own call volume.
As a worked example, here is what Priya costs at different volumes. The plan is ₹6,999 a month with 450 minutes included, then ₹5 a minute, with no setup fee. The table assumes 1.5 minutes a call.
| Calls a day | Minutes a month | Monthly cost | Cost per call |
|---|---|---|---|
| 10 | 450 | ₹6,999 | ₹23 |
| 20 | 900 | ₹9,249 | ₹15 |
| 30 | 1,350 | ₹11,499 | ₹13 |
Questions to ask every vendor about price:
- Is there a setup fee, and what does it cover?
- How many minutes are included, and what is the rate after that?
- Are outgoing calls priced the same as incoming calls?
- Is the phone number and call forwarding included?
- Is GST included in the quoted price?
- Can you cancel monthly, or is there a lock-in?
The cost only makes sense next to what missed calls cost you. Our free missed-call cost calculator works this out from your own call log.
How is it different from IVR, voicemail or a call-back routine?
Clinics already have ways to deal with calls they cannot pick up. Here is how they compare on the things that decide whether a caller books.
| Option | Caller talks to | Books on the call? | Works after hours? | Main weakness |
|---|---|---|---|---|
| Phone rings out | Nobody | No | No | Caller tries another clinic |
| Voicemail | A recording | No | Yes | Few callers leave a message |
| IVR menu ("press 1") | A menu | Rarely | Yes | Callers give up in menus |
| Call-back routine | Staff, later | Later, if reached | No | By the time you call back, they have booked elsewhere |
| AI receptionist | A conversational AI | Yes | Yes | Can mishear; needs up-to-date clinic facts |
Why language matters so much in India
Many AI phone systems are built mainly for English callers. Indian clinics get calls in Hindi, Kannada, Telugu, Tamil, Malayalam and more, often mixed with English in the same sentence: "Doctor ka appointment kal evening mil sakta hai?"
An AI receptionist for India needs to:
- recognise the caller's language from their first words and reply in it,
- keep common English words like "appointment", "slot" and "doctor" as callers say them,
- not switch language because one word was misheard,
- handle Indian names and read them back for confirmation.
The only reliable test is to call the system yourself in the languages your patients use. Priya speaks English, Hindi, Kannada, Telugu, Tamil, Malayalam; you can try her on +91 94849 57261.
Is it legal to use an AI receptionist for patient calls?
Using software to answer calls is legal, but patient calls involve personal data, so India's Digital Personal Data Protection Act, 2023 applies. In practice, a clinic should:
- tell callers that the call may be answered by an AI and recorded,
- collect only what is needed to book: name, number, preferred time and reason for visit,
- know where recordings and bookings are stored and who can see them,
- be able to delete a patient's data when asked.
Ask any vendor for their privacy policy and how deletion requests work. Ours is in our privacy policy. This section is general information, not legal advice.
How to choose an AI receptionist: 10 checks
We use this checklist ourselves when we test Priya. Run each check by calling the demo line, not by reading the website.
- Speed. Does it answer on the first ring and reply without long pauses?
- Your languages. Call in each language your patients use, including mixed sentences.
- Names. Give an uncommon name. Does it confirm it correctly?
- Dates and times. Ask for "next Tuesday evening". Does it book the right date?
- Changes. Reschedule and then cancel on the same call.
- Medical questions. Ask "Which medicine should I take?" It should refuse and offer to book or pass a message.
- Unknown questions. Ask something it cannot know. Does it say so, or make something up?
- Noise. Call from a noisy place. Does it get confused or interrupt?
- Records. Where does the booking appear, and can your team use it without new software?
- People behind it. Who fixes mistakes, and how quickly?
How to set one up
Setup for a clinic usually takes a few days, most of it gathering the right information:
- List your facts. Doctors and their days, timings, address and landmark, treatments, and which prices may be shared.
- Set booking rules. Slot length per visit type, buffer time, and how far ahead patients can book.
- Decide what goes to a person. Complaints, billing questions and anything clinical.
- Test on your own details. Call it as a patient several times before going live.
- Choose forwarding. All calls, only when busy, or only after hours. Many clinics start with after hours.
- Review the first week. Read the call records and correct anything it got wrong.
Common questions
Will patients know they are talking to an AI?
They should. Being open about it builds trust, and the greeting can say so. Many callers are mainly interested in getting their appointment booked quickly.
Which calls should an AI receptionist answer first?
Start with the calls nobody can pick up today: after hours, during procedures and when the line is busy. Add daytime calls once you are happy with how it books.
Can it work with my clinic software?
It depends on the vendor and the software. Priya writes bookings to a Google Sheet, which works for any clinic without new software.
How long are typical calls?
Most booking calls take one to two minutes. Calls with many questions about a treatment take longer.
What is the best AI receptionist for clinics in India?
The one that handles your patients' languages, books correctly and never gives medical advice. Use the 10 checks above on each option. We built Priya for Indian clinics, so we are not neutral; call +91 94849 57261 and judge for yourself.
